  2. Pensioner card having photograph of the applicant.
  3. Central Government Health Scheme Card.
  4. Ex-Servicemen Contributory Health Scheme photo card.
  5. Certificate of identity in Original signed by a Gazetted officer.
  6. Arm’s license.
  7. Passport.
  8. Driving License.
  9. Bank certificate in Original on letter head from the branch(along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant.
  10. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.
  11. Aadhaar Card.
  12. Ration card having photograph of the applicant.
  13. Certificate of identity in Original signed by a Member of Parliament.
  14. Election Card / Voter ID.
  15. Certificate of identity in Original signed by a Member of Legislative Assembly.
Birth Date Proof (Any one):
  1. Passport.
  2. Matriculation certificate or Mark sheet of recognized board.
  3. Driving License.
  4. Aadhaar Card.
  5. Birth certificate issued by the Municipal Authority or any office authorised to issue birth.
  6. Affidavit sworn before a magistrate stating the date of birth.
  7. Central Government Health Scheme Card.
  8. Ex-Servicemen Contributory Health Scheme photo card.
  9. Domicile certificate issued by the Government.
  10. Election Card / Voter ID.
  11. Pension payment order.

Is PAN Card Duplicate Copy required?

Financial Institutions or Banks ask to give PAN Card as PAN proof. If PAN Card is not present with you or not found, you can apply for PAN Card Duplicate Copy and present Duplicate PAN Card.
