  6. Certificate of identity in Original signed by a Member of Legislative Assembly.
  7. Certificate of identity in Original signed by a Member of Parliament.
  8. Ration card having photograph of the applicant.
  9. Certificate of identity in Original signed by a Gazetted officer.
  10. Election Card / Voter ID.
  11. Passport.
  12. Pensioner card having photograph of the applicant.
  13. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.
  14. Certificate of identity in Original signed by a Municipal Councilor.
  15. Central Government Health Scheme Card.
Birth Date Proof (Any one):
  1. Central Government Health Scheme Card.
  2. Birth certificate issued by the Municipal Authority or any office authorised to issue birth.
  3. Ex-Servicemen Contributory Health Scheme photo card.
  4. Domicile certificate issued by the Government.
  5. Matriculation certificate or Mark sheet of recognized board.
  6. Driving License.
  7. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.

  9. Affidavit sworn before a magistrate stating the date of birth.
  10. Pension payment order.

Is PAN Card Duplicate Copy required?

Financial Institutions or Banks ask to give PAN Card as PAN proof. If PAN Card is not present with you or not found, you can apply for PAN Card Duplicate Copy and present Duplicate PAN Card.
