  2. Bank certificate in Original on letter head from the branch(along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant.
  3. Certificate of identity in Original signed by a Member of Legislative Assembly.
  4. Driving License.
  5. Pensioner card having photograph of the applicant.
  6. Election Card / Voter ID.
  7. Ration card having photograph of the applicant.
  8. Aadhaar Card.
  9. Central Government Health Scheme Card.
  10. Passport.
  11. Certificate of identity in Original signed by a Member of Parliament.
  12. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.
  13. Ex-Servicemen Contributory Health Scheme photo card.
  14. Arm’s license.
  15. Certificate of identity in Original signed by a Gazetted officer.
Birth Date Proof (Any one):
  1. Aadhaar Card.
  2. Passport.
  3. Driving License.
  4. Domicile certificate issued by the Government.
  5. Central Government Health Scheme Card.
  6. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.
  7. Election Card / Voter ID.
  8. Matriculation certificate or Mark sheet of recognized board.
  9. Marriage Certificate.
  10. Ex-Servicemen Contributory Health Scheme photo card.
  11. Affidavit sworn before a magistrate stating the date of birth.
  12. Pension payment order.
  13. Birth certificate issued by the Municipal Authority or any office authorised to issue birth.

  16. Allotment letter of accommodation issued by the Central/State Government less than 3 years old.

  19. Bank account statement less than 3 months old.
  20. Domicile certificate issued by the Government.
  21. Landline/Broadband connection bill less than 3 months old.

Is PAN Card Duplicate Copy required?

Financial Institutions or Banks ask to give PAN Card as PAN proof. If PAN Card is not present with you or not found, you can apply for PAN Card Duplicate Copy and present Duplicate PAN Card.

Who has to sign the Duplicate PAN Application?
